On July 2, 2020, at 2:00 p.m., an employee was unloading freight at a store when a rollatini measuring 2' x 4' fell on his leg. The rolltainer was on a lift gate and became unlevel, tipping over and striking the employee's ankle. The employee was hospitalized with a closed, non-displaced fracture of the medial malleolus of the right tibia that required surgery.

An employee was measuring the thickness of a brass strip as it was being fed into a press. A micrometer the employee was using was pulled into a 4-inch gap at the press opening, taking the employee's right hand with it. The employee's little finger was partially amputated.

An employee was assigned to install a small hydraulic cylinder at 6 Clean #1 Payoff hold down roll. The employee had the cylinder in place on the blank end clevis with the pin inserted. While the employee was lining up the rod end clevis, their right hand pinky finger was partially inserted in the pin hole. When the cylinder dropped, it struck the tip of the employee's gloved finger resulting in amputation of an eighth of an inch of the finger. The amputation did not include bone.

An employee was finishing up repairs to the 4DC Up Elevator. The employee proceeded to pull slack out of the drive chain. After cleaning the teeth of the sprocket, the weight of the chain pinched the employee's finger between the chain and a sprocket resulting in the amputation of 3/8 inch of the tip of the left right finger. LOTO was used.
